I've found the description of the flag of Horní Bezděkov (Kladno district, Central Bohemian region, Czechia) at this website:
"Popis praporu: List tvorí tri vodorovné pruhy, zelený, žlutý a zelený v pomeru 1 : 3 : 1."
That is: a field with three horizontal stripes green - yellow - green, proportions 1:3:1. Flag adopted 14 Dec 1998.
The colors are of course from the municipal arms.
