At http://www.gobernacioncaaguazu.ya.st/ it can be seen, at the beginning of the Flash presentation (hard to see sometimes because of the advertising pop-up), a flag which obviously must be the departmental flag since it appears too in the top banner of the website.
It is a horizontaly divided flag, in the national colors (RWB), proportions
3:5, 1+2+1. In the centre, there are five green stars, the central one being
bigger.
The 5 stars may stand for he fact that the department is known as #5 in
paraguayan territorial organization.

According to Jaume Ollé's Flag Report [frp]
(issue 23):
"The flag of Caaguazu is blue and contains the shield. It does not has
official approval (Communication of Alfredo - Emilio Cantero Lizarrazu dated
14 October 2000.)"
