A white flag with the CMA logo on it: on a partial blue globe, a cross, crown and communion jug and goblet.

The Christian and Missionary Alliance (CMA) is a small but extremely active denomination which has engaged in missionary activities around the world for over a century. There is no actual illustration of its denominational logo on its home page, although a monumental version is visible in the banner. An explanation of the symbols used in the denominational logo can be found at http://www.cmalliance.org/resources/logo/logo.jsp

I don't know how widely the flag is used. I spoke to a representative of the organization who explained that they are very protective of their trademark and do not want the image misused.
